New Patients

What to expect

Please arrive 15-30 minutes before your appointment time to allow time to complete front desk check-in process.

Next your nurse will call you back to record vital signs, review your health history and then inform your doctor of your arrival.

Then your neurologist will perform a detailed history and physical exam. They will review possible diagnosis, testing and treatment options.

Finally you will receive follow up instructions and a written after visit summary before proceeding to the check-out desk.

Health records, notes, including imaging reports, discs, films

Insurance Filing

As a courtesy we will file your charges with your primary health insurance carrier. Depending on your insurance plan and coverage, you may be responsible for unmet deductibles and co-payments at the time of service. Inability to make co-payments at time of service will result in appointment rescheduling.

Self-paying and uninsured patients are required to bring a $200 payment, and then will be billed the balance depending on the services rendered.

Motor Vehicle Accidents/Litigation

We do not file with automobile insurance carriers, nor do we hold bills at the request of attorneys who handle motor vehicle accidents or other litigation. Payment for services rendered under these circumstances is due in full at the time of service. We will be glad to provide you with a statement of your charges so that you can receive reimbursement for your payment.


$50 Rescheduling fee: No shows or cancellations within 24 hours will result in rescheduling fee, to be collected before next appointment. Repeated cancellations or no-shows will result in dismissal from the practice.

$50 Form completion fee: Outside forms will be reviewed by physicians and staff. If they agree to complete the form, then payment will be collected at time of pickup or delivery.
